Conditions of Use

CUA Health Online Services allows you to:

• Register for use.

• View/change your membership details.

• View your claims history and limits usage.

• View rates quote and premium calculator.

• View/print tax statements.

• Apply for CUA Health membership

• Make a one-off premium payment by credit card

• Receive certain correspondence electronically

• Order a new membership card

You must have a valid email address to access Online Services. By registering for Online Services you agree to be bound by the conditions of use. Please refer to the conditions of use each time you visit Online Services as we may update them from time to time.

You are required to keep your password private, as these services are provided for the primary member only. Only one password is allowed per membership. Therefore, you (the member) are responsible for all activities undertaken on the membership. It is your responsibility to take reasonable steps to ensure security of your password and to notify CUA Health immediately if your password is lost or stolen.

Availability

We will endeavour to make the system available 24 hours a day, seven days per week. However, due to unforeseen circumstances this is not guaranteed.

Communications

By registering for Online Services you accept that communications to you from CUA Health will be in an electronic format. The exceptions to this is regulatory communications and the initial Online Services Registration Letter, which will contain your activation code, and will be forwarded by standard mail.

Your choice of communication method consists of:

Webmail - authorises CUA Health to hold communications in a secure depository for you to view/print after you have logged into Online Services. These communications are private and can only be viewed by you. We will notify you via email or SMS of any communications held for viewing.

Email - authorises CUA Health to forward any communications in full to your registered email address.
